Partnering with Mattel requires that retailers complete the Mattel customer account application on the next few pages, and that Mattel accepts the application. The application asks for specific information regarding your retail establishment. All sections denoted with an * must be completed. Please allow 20 – 30 minutes to complete this form.

Potential retail customers must meet the following requirements:

1. 1.Must be legitimate retail operation with retail store front, or acceptable web based business as determined by Mattel, Inc. Web based retailers click here for requirements of new web based customers.
2. 2.Store front, commercially zoned dedicated warehouse and customer network must be based within the 50 states of the United States of America, the District of Columbia, Puerto Rico, and Guam ("the United States").
   The following warehouse/ warehouse locations are not considered appropriate:
            A. UPS store or Post Office Box location.
            B. Warehouse, garage, structure or barn on residential property.
            C. Any fulfillment center.
            D. The warehouse may not be shared by any other business.
            E. Storage rental facility or unit.
            F. Freight Forwarding Address.
   
3. 3.Must have state specific resale certificates for each state that receive shipments.
4. 4.Must meet minimum opening order/reactivation requirement of $50,000.00, minimum re-order requirement of $1,000.00, and total annual shipping volume must reach $50,000.00.

Please be prepared to provide the following information during the application process:

1. 1.Corporate contact information (company name, address, phone and fax numbers, as well as email address). Please ensure you are giving accurate corp information so we can attain credit history.
2. 2.State specific resale certificate. You must provide a resale certificate for each state you ship to prior to your first order.
3. 3.Ship to/Bill to address: include primary ship to address at this time. (Additional ship to addresses may be added once an account is established. All ship to addresses must meet requirements and be able to supply resale certificates)
4. 4.Trade references for companies you CURRENTLY do business with. (Must include fax or email address.)
5. 5.Estimated amount of first order and estimated annual sales volume.

The credit and personal data supplied will be encrypted using Secure Socket Layer (SSL) technology, and cannot be read as it travels across the Internet. Additionally, all credit and personal data will be encrypted in the Mattel database and will not be available to non-Mattel personnel.
